Girl Registration Form
Tips for filling out the Girl Registration Form:
-Print out 2 copies of the registration form. Print one for your records and one to SIGN and mail.
-Include your Service Unit (SU) number on your registration. You can find your number using our Service Unit Locator .
-Contact us with questions. If you have any questions or confusion when completing the registration form, contact your local service center. By answering your questions before your registration form is sent in, we can expedite your registration process. We are here to help you!
-Financial assistance is available - application form #116b is listed below under Troop > Financial forms.

Financial Assistance Request - Travel (#116d) Use this form to request financial assistance for council-sponsored travel opportunities (not destinations or troop trips). Financial assistance is limited to available funds. Preference will be given to girl members in awarding assistance. Chaperones may apply and will be awarded funds after girl requests have been fulfilled.

Notice of Intent - Troop Travel (#119) This form replaces the following legacy forms: Travel Progression 1 and 2, Activity Roster for Field Trips and General Activity Roster; Notice of Intent - Short Trip; Intent to Go; Application for Day Trips, Overnights, and Camping. Please consult the new Policies and Procedures - out soon - for further explanation. Please direct questions on this change to your local program specialist .
