Cannot attend more than 1 meeting before providing registration form and paying GSUSA membership dues ($12). Membership year is October 1 – September 30. Financial assistance is available.

Tips for filling out the Girl Registration Form:

  • Spring Re-registration FAQ   
  • Print out 2 copies of the registration form. Print one for your records and one to SIGN and mail.
  • Include your Service Unit (SU) number on your registration. You can find your number using our Service Unit Locator .
  • Contact us with questions. If you have any questions or confusion when completing the registration form, contact your local service center . By answering your questions before your registration form is sent in, we can expedite your registration process. We are here to help you!
  • Financial assistance is available - application form #116b is listed below.

This form replaces the following legacy forms: Travel Progression 1 and 2, Activity Roster for Field Trips and General Activity Roster; Notice of Intent - Short Trip; Intent to Go; Application for Day Trips, Overnights, and Camping.
